During the checkout process parents will be asked for their contact information and information on the child(ren) they will be attending with. They will also be asked to detail anything else you might need to know (known allergies, medical conditions).
Providers are not able to add bespoke questions. However, this is something we are looking at for the future.
You can however add links to your bespoke confirmation email and so if you have online forms that they need to complete, then you can add those in. Find out how to customise your emails here.
